New 'Annotations' feature added to CLWRota and Medirota

The Trainee Exception Reporting feature has been extended to allow all clinicians to make a number of different notes on any session. This feature is now called Annotations.
Clinicians can add annotations to a session via their web login using the new Annotations tab or via the mobile app by clicking into the session.
Annotation reports can be generated by both clinicians and rota masters (where annotations are public). This feature could be used to track when sessions overrun, for example.
Annotation types can be created in the super user area. They can either be private - only visible to the clinician who made the annotation, or public - visible to both rota masters and the clinicians who made them.
